NOTE:  All applicable fields must completed, or the online appeal will not be processed.  An email address must be included.  All notifications will be via email.  If an email address is not provided, the appeal will not be processed.  An appeal fee of $5.00 will be charged for all denied appeals.  Appeal fees are not charged for approved appeals.  If multiple citations are being appealed, complete one form for each citation.

Appellants will be notified of the upcoming schedule of appeal hearings.  Appellants must appear at scheduled hearings, or their citation appeals will be automatically denied and a $5.00 appeal fee charged.  Appellants are encouraged to take evidence to the appeal hearings such as personal witnesses, photographs, etc.
